You will work collaboratively with service users, carers, and professionals to promote independence, dignity, and wellbeing in line with Care Act principles.
Key Responsibilities:

Complete strengths-based assessments and support plans under the Care Act 2014, focusing on promoting independence and wellbeing.

Undertake safeguarding enquiries in accordance with Section 42 responsibilities, ensuring the protection of vulnerable adults.

Work in partnership with health professionals, housing teams, and voluntary services to deliver integrated care.

Apply the Mental Capacity Act 2005 in assessing decision-making capacity and arranging best interest meetings when necessary.

Maintain accurate case records, ensuring compliance with statutory duties, council procedures, and performance targets.
