View JobNorth Lincolnshire Council is seeking a dedicated and proactive Hospital Discharge Social Worker to support timely and effective discharges from acute and community hospitals. This role is vital in ensuring that adults receive the right care, at the right time, in the most appropriate setting. Key Responsibilities:

Coordinate hospital discharges by conducting timely assessments and arranging care packages in line with the Care Act 2014 and Discharge to Assess (D2A) principles.

Work collaboratively with NHS discharge teams, community services, and families to ensure safe transitions from hospital to home or appropriate care settings.

Undertake safeguarding enquiries and mental capacity assessments, ensuring all statutory duties are fulfilled with a focus on person-centred outcomes.

Maintain accurate case records and contribute to performance targets, supporting continuous improvement in hospital discharge pathways and reducing delayed transfers of care.
