The ideal candidate will have a solid understanding of relevant legislation, excellent assessment skills, and a commitment to delivering person-centred care.
Key Responsibilities:

Carry out Care Act 2014 assessments to identify the needs of older adults and develop tailored support plans.

Undertake safeguarding enquiries under Section 42 to protect vulnerable older adults from abuse or neglect.

Collaborate with families, carers, health professionals, and voluntary sector partners to coordinate effective support.

Apply the Mental Capacity Act 2005 when assessing decision-making capacity and supporting best interest decisions.

Maintain accurate, timely records and contribute to reviews and care planning in accordance with Harrow Council policies and statutory requirements.
