Key Responsibilities:

  • Lead and manage a team of social workers and practitioners focused on hospital discharge and post-discharge support.

  • Oversee complex casework, ensuring Care Act compliance and effective multi-agency coordination.

  • Drive performance improvement, ensuring timely assessments and discharges within legal and NHS framework requirements.

  • Provide supervision, mentorship, and support to staff, fostering professional development and resilience.

  • Maintain quality assurance, service delivery standards, and adherence to safeguarding procedures.
