Job details: Occupational Therapist required to be responsible for:
- Professional assessments and care interventions within legislation and the COT and HCPC practice requirements
- Manage, supervise and support staff as required.
- Contribute the skills, knowledge and performance of the OT team
JD available on request:
- To provide advice and information relating to the provision of Social Care for Adults / Older People / Children. Undertake social care assessment, care planning, monitoring and review service for service users with highly complex needs within the Greenwich Prison Cluster, using an integrated case management approach.
- To provide advice and information under the Care Management/Occupational Therapy Process for Adults / Older People / children which could lead to service provision or referral on to other departments, agencies and/or organisations
- Interventions aim to enhance the person’s independence, safety and quality of life via rehabilitation, Moving & Handling, local / out of borough hospitals / voluntary sector specification of equipment and/or structural changes to the environment.
- To liaise and promote the continuity of service provision.
- To undertake appropriate safeguarding work within Adults / Older People / children.
Working hours: 35 hours per week. 09.00 – 17.00.
