Independent assessment, treatment, and discharge planning for patients in a community rehab setting

Use of clinical reasoning and recognised outcome measures to guide therapy interventions

Management of own caseload and treatment planning with confidence and autonomy

NHS experience required; post-qualification experience in community or rehab settings is preferred

Car drivers preferred due to community-based visits; agile working supported
