Job Title: Occupational Therapist – Band 6

Service: Intensive Rehabilitation & Therapy Service

Job Details:

  • Band 6 Occupational Therapist required to join the Intensive Rehabilitation & Therapy Service within a community rehabilitation team.
  • Undertake independent assessment, treatment and discharge planning for patients in community rehabilitation settings.
  • Use recognised outcome measures to guide treatment planning and monitor progress.
  • Assess, recommend and order appropriate equipment to support independence and safe discharge.
  • Facilitate family meetings and contribute to multidisciplinary care planning.
  • Work collaboratively with healthcare professionals to support rehabilitation and safe transitions of care.
  • Maintain accurate and timely clinical documentation in line with NHS standards.

Skills / Experience:

  • HCPC registered Occupational Therapist.
  • Post-qualification experience within the NHS.
  • Experience in community rehabilitation settings (desirable).
  • Experience with equipment assessment and ordering.
  • Ability to work autonomously and manage a caseload.
  • Strong multidisciplinary team working and communication skills.
  • Full UK driving licence (essential due to community-based work).

Working Hours: 37.5 hours per week, Monday to Friday, 08:30 – 16:30. Part time considered (minimum 3 days / 22.5 hours).
