Job Overview
We are seeking a dedicated Band 6 Occupational Therapist to join the Intensive Rehabilitation and Therapy Service at NELFT – Grays Court Community Hospital in North East London. This locum role involves working within the community rehabilitation team, providing independent assessment, treatment, and discharge planning for patients requiring rehabilitation support. The successful candidate will use evidence-based practice, clinical reasoning, and outcome measures to deliver effective therapy and support patient recovery and independence at home.
Key Responsibilities

- Conduct comprehensive assessments, develop treatment plans, and manage discharge planning for patients within the community rehabilitation team.
- Apply clinical reasoning and recognised outcome measures to guide therapeutic interventions and track progress.
- Work independently to manage and prioritise your own caseload, delivering high-quality, patient-centred rehabilitation.
- Collaborate with multidisciplinary colleagues including physiotherapists, nurses, and social care professionals to ensure coordinated and holistic care.
- Support patients in achieving rehabilitation goals, promoting independence and functional recovery within their home or community environment.
- Maintain accurate, timely, and professional documentation in line with NHS standards and trust policies.
About You
You are an experienced and confident Occupational Therapist who thrives in a community-based rehabilitation environment. You’ll be skilled in independent caseload management, adaptable in approach, and committed to achieving the best outcomes for your patients. A good understanding of evidence-based interventions and the ability to apply clinical reasoning effectively will be essential. A valid UK driving licence would be an advantage given the community nature of this role.
Requirements

- Degree or diploma in Occupational Therapy and current registration with the Health and Care Professions Council (HCPC).
- Experience working in the NHS post-qualification, ideally within community rehabilitation or similar clinical settings.
- Strong knowledge of rehabilitation principles, outcome measures, and discharge planning.
- Excellent organisational and communication skills, with the ability to manage your own caseload effectively.
- Commitment to person-centred, evidence-based practice.
- Full UK driving licence and access to a vehicle preferred.
