Occupational Therapist – Band 6 (Locum)

Job Overview

We are recruiting an experienced Band 6 Occupational Therapist to join the Intermediate & Frailty Team at Halton Borough Council, within Buckinghamshire Social Services. This locum role focuses on providing specialist assessment, intervention, and rehabilitation for older adults with complex and/or frail health needs. You’ll work closely with a multidisciplinary team to promote independence, wellbeing, and safe living in the community through high-quality, person-centred occupational therapy practice.

Key Responsibilities

•   
  • Conduct comprehensive occupational therapy assessments for adults with frailty and complex health needs.
     

  • Develop, implement, and review individualised care plans and rehabilitation programmes.
     

  • Provide advice and education to patients, carers, and professionals on appropriate equipment, adaptations, and strategies to support independence.
     

  • Work collaboratively with health and social care professionals to ensure seamless and coordinated care pathways.
     

  • Support adults to remain safely in their own homes and communities.
     

  • Contribute to service development, quality improvement, and the promotion of best practice within the team.
     

  • Maintain accurate and timely clinical documentation in line with council policies and professional standards.

   

  About You

   

  You are a confident and compassionate Occupational Therapist with experience supporting older adults and individuals with complex needs. You bring strong clinical reasoning and decision-making skills, alongside excellent communication and teamwork abilities. Your person-centred approach and commitment to promoting dignity, independence, and wellbeing will enable you to make a meaningful impact within the Intermediate & Frailty service.

   

  Requirements  

  • • Degree or diploma in Occupational Therapy and current registration with the Health and Care Professions Council (HCPC).
       

    • Experience working within adult or older adult services.
       

    • Experience within frailty, rehabilitation, or intermediate care services is highly desirable.
       

    • Strong clinical reasoning, assessment, and decision-making skills.
       

    • Excellent communication, interpersonal, and team-working abilities.
       

    • Commitment to person-centred, strengths-based, and outcome-focused practice.
