- Care Planning and Coordination: Develop personalised care plans in partnership with individuals, families, hospital staff and other professionals, identifying appropriate care options such as home care, residential care or rehabilitation services.
- Discharge Planning: Lead or contribute to discharge planning meetings to ensure safe and timely discharge, arranging equipment, adaptations and ongoing care services as required.
- Safeguarding: Identify and respond to safeguarding concerns, working with adult safeguarding teams and statutory bodies to protect individuals’ safety and wellbeing.
- Advocacy and Support: Act as an advocate to ensure individuals’ views and preferences are heard, providing information and guidance to individuals and families navigating the social care system.
