Key Responsibilities:

Deliver comprehensive physiotherapy assessments and evidence-based interventions for children and young people across community settings.

Provide specialist advice on and facilitate the provision of mobility aids, postural management equipment, and physical adaptations to support functional independence.

Develop and implement personalised care plans in collaboration with families, schools, and multi-disciplinary colleagues.

Monitor progress and adjust intervention plans in response to clinical need, ensuring goals are realistic, achievable, and child-focused.

Support parents and carers with education and training to optimise outcomes in the home and school environments.

Maintain clear and accurate clinical records in accordance with Trust policies, and contribute to audits, service development, and professional learning.
