Based at Wexham Park Hospital, part of Frimley Health NHS Foundation Trust, this role sits within the Older People and Integrated Care Service, providing specialist Occupational Therapy assessment, rehabilitation, and discharge planning within a busy acute hospital environment.

The successful candidate will work across Older People’s Services, General Medicine, and the Emergency Department, supporting patients with complex rehabilitation needs to maximise independence and facilitate safe, timely discharge from hospital. This opportunity would also suit an experienced Band 6 Occupational Therapist with substantial acute NHS experience who is ready to work at Band 7 level.

Key Responsibilities

  • Manage a complex caseload across Older People and Integrated Care pathways.
  • Deliver evidence-based rehabilitation programmes to maximise functional recovery and independence.
  • Undertake comprehensive functional, cognitive, and environmental assessments.
  • Lead complex discharge planning to ensure safe and effective transitions from hospital to community services.
  • Assess and recommend complex manual handling equipment and rehabilitation solutions.
  • Work flexibly across General Medicine and the Emergency Department to meet service demands.
  • Collaborate with multidisciplinary teams, patients, carers, and external agencies to coordinate patient care.
  • Provide advice and education to patients and families throughout the rehabilitation process.
  • Maintain accurate clinical documentation in accordance with HCPC and NHS standards.
  • Support service development and contribute to quality improvement initiatives.

About You

You will be an experienced Occupational Therapist with a strong background in acute NHS hospital practice, particularly within rehabilitation, older people’s services, and discharge planning. You will be confident managing complex caseloads and working autonomously within a fast-paced multidisciplinary environment.

Excellent clinical reasoning, communication, and organisational skills are essential. Applications are welcomed from experienced Band 6 Occupational Therapists with the relevant acute NHS experience.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Significant experience working within a UK NHS hospital setting.
  • Acute Occupational Therapy experience within rehabilitation and discharge planning.
  • Experience working with older adults and patients with complex medical needs.
  • Knowledge of complex manual handling equipment and rehabilitation techniques.
  • Experience within General Medicine and/or Emergency Department services is desirable.
  • Excellent assessment, clinical reasoning, and communication skills.
  • Ability to work independently and effectively within multidisciplinary teams.
  • Experienced Band 6 Occupational Therapists with appropriate NHS experience will also be considered.
