- Occupational Therapist required to join the Minor Aids & Adaptations Team
- Work within a fast-paced environment where the team aims to complete assessments and cases within approximately 15 working days
- Promote strengths-based practice to support independence, health and wellbeing
- Carry out needs-led assessments in line with the Care Act for residents and their carers
- Provide Occupational Therapy services to individuals with physical disabilities and complex needs, including those with mental health conditions
- Work as part of a multidisciplinary team to assess needs and develop appropriate intervention plans
- Use professional judgement within relevant legal and policy frameworks
- Plan and implement interventions that support independence and reduce or delay the need for ongoing care
- Promote cost-effective and efficient use of resources in assessment and intervention
- Assess carers’ needs and provide support where eligible
