We are seeking a Band 6 RMN, Social Worker, or Occupational Therapist to provide mental health care within the community mental health team (ICMHT) in Redruth, Cornwall. This role involves managing a caseload of mental health patients, providing assessments, interventions, and support.
Key Responsibilities:
Manage a caseload of community mental health patients.
Conduct assessments, develop care plans, and implement interventions.
Work collaboratively with multidisciplinary teams to support patient recovery.
Engage with external agencies to ensure a holistic approach to care.
Maintain accurate clinical documentation in line with NHS policies.
Requirements:
Registered Mental Health Nurse (RMN), Social Worker, or Occupational Therapist.
Community Mental Health experience is essential – must be clearly stated on CV.
Full UK driving licence and own vehicle required (mileage reimbursed for patient visits).
Strong assessment, communication, and decision-making skills.
