CPN Care Coordinator – Early Intervention
Tripod Partners are seeking an experienced Community Psychiatric Nurse (CPN) Care Coordinator to join the Essex Support & Treatment in Early Psychosis (ESTEP East) team. This is a full-time role providing care coordination for individuals experiencing early psychosis.
Key Responsibilities:
Provide care coordination and support for individuals experiencing Early Intervention in Psychosis (EIP).
Conduct assessments, develop care plans, and deliver interventions tailored to patient needs.
Work closely with multi-disciplinary teams to ensure effective and holistic patient care.
Position Details:
Location: Laurel House, Rochford Hospital, Essex
Work Pattern: Monday to Friday, 9:00 AM – 5:00 PM
Rate: £26.87 per hour
Pay Type: Umbrella or PAYE available
Contract Length: ASAP start until 3rd April 2025
Requirements:
Registered Mental Health Nurse (RMN) with experience in Early Intervention in Psychosis (EIP).
Strong assessment and care coordination skills.
Previous experience working within a community mental health setting.
