Band 6 CPN Care Coordinator – Early Intervention Team
Tripod Partners are currently seeking a skilled Band 6 Community Psychiatric Nurse (CPN) to join the Essex Support & Treatment in Early Psychosis (ESTEP) team. This role is based at Laurel House, Rochford Hospital, and focuses on providing care coordination for early intervention in psychosis.
Key Responsibilities:
Act as a Care Coordinator for individuals within the Early Intervention pathway.
Deliver comprehensive mental health assessments and care planning.
Provide therapeutic interventions tailored to patient needs.
Work collaboratively with a multi-disciplinary team to ensure seamless patient care.
Registered CPN with experience in mental health care coordination.
Previous experience working within Early Intervention or similar services.
Strong skills in mental health assessment, care planning, and delivering interventions.
