Community Psychiatric Nurse – Older Adults Mental Health Team

Tripod Partners is seeking a Community Psychiatric Nurse (CPN) with experience in older adults’ mental health to join the South Bucks Older Adults CMHT based at Saffron House, High Wycombe. This is a full-time, face-to-face role involving key working and duty desk responsibilities.

Position Details

• Location: Saffron House, Easton St., High Wycombe, HP11 1NH
• Pay Rate: £26.00 per hour
• Work Pattern:
  • Monday to Friday, 8:00 AM to 5:00 PM
  • Must be available on Thursdays for MDT meetings
• Duration: ASAP to 30/04/2025
• Hours: 37.5 hours per week

Key Responsibilities

• Conduct mental health assessments and provide interventions for older adults, including those with dementia.
• Perform physical health reviews and administer depot injections as required.
• Manage a caseload and take on duty desk responsibilities.
• Collaborate with the multi-disciplinary team (MDT) to ensure high-quality, person-centred care.

Requirements

• Registered Community Psychiatric Nurse (CPN) with experience in older adults’ mental health.
• Strong skills in mental health assessment, care planning, and dementia care.
• Confident in performing physical health reviews and administering depots.
• Must have a valid driving license and be able to travel to High Wycombe for in-person duties.
• Please note: This is not a remote-working role as it involves face-to-face assessments.
