Job overview:
A Band 6 Physiotherapist is required to start as soon as possible within the Falls Team.
This is a community-based role covering Hull and the East Riding area, supporting patients
who have experienced falls and are at risk of further incidents.
Main duties and responsibilities:

- Provide specialist physiotherapy assessment and intervention for patients who have experienced falls
- Deliver community-based physiotherapy services aimed at reducing falls risk and promoting independence
- Assess mobility, balance, strength and functional ability, and develop tailored rehabilitation programmes
- Work collaboratively within the multidisciplinary Falls Team to support holistic care planning
- Provide education and advice to patients and carers to support falls prevention
- Maintain accurate and timely clinical records using SystmOne
- Travel across Hull and East Riding to complete community visits as required
Skills and experience required:

- Degree or equivalent qualification in Physiotherapy
- Current professional registration
- Essential community physiotherapy experience
- Experience working with patients who have experienced falls
- Experience using SystmOne (essential)
- Ability and willingness to travel across Hull and East Riding
- Strong communication, clinical reasoning and organisational skills
