Job Overview
Information and Assessment Officer required to support Adult Social Care services, delivering high-quality, customer-focused information, advice, and initial conversations to Hackney residents and their carers.
Key Responsibilities
-
- Work with Social Workers and other professionals within Adult Social Care to provide effective, accessible information and Initial Conversations.
- Deliver a high standard of customer care, offering advice and assistance on council services and services provided by statutory, independent, and voluntary agencies.
- Carry out initial conversations using a strengths-based approach to identify needs and promote independence.
- Act as a Trusted Assessor, providing simple equipment to older and disabled people to help them maintain independence.
- Produce individually tailored plans for simple equipment provision.
- Recommend and requisition Telecare services where appropriate.
- Support the setup of social care services following assessment.
