Job overview:
An experienced Physiotherapist is required to join the Integrated Discharge Service (IDS) in Islington.
The IDS is a dynamic, fast-paced, 7-day service that supports residents to return home safely from hospital.
This role is central to assessing risk, designing interventions, and supporting people at different stages
of the hospital discharge pathway using a strengths-based approach to achieve positive outcomes.
The role offers flexible working arrangements, combining home working, office-based duties, and community visits.
Main duties and responsibilities:

- Support the Discharge to Assess (D2A) pathway through screening referral documentation and contributing to decision-making around safe discharge planning
- Carry out post-discharge assessments in people’s homes, providing physiotherapy interventions to support independence, recovery and prevent hospital admission
- Assess environmental factors and recommend appropriate equipment, minor adaptations and alternative strategies for managing daily activities
- Provide manual handling advice and recommendations to promote safety and independence
- Work with residents on discharge pathways to design personalised Reablement Plans and goal-focused interventions
- Apply a strengths-based approach, enabling people to maximise their abilities and achieve agreed outcomes
- Work collaboratively within a multidisciplinary team across health and social care services
- Maintain accurate, timely and professional documentation in line with service and statutory requirements
Key requirements:

- Qualified Physiotherapist with relevant professional registration
- Experience working within discharge, reablement, community or integrated health and social care services
- Strong clinical reasoning skills with the ability to assess risk and plan effective interventions
- Confidence working autonomously in the community as well as within a multidisciplinary team
- Commitment to strengths-based, person-centred practice
- Good communication, organisational and documentation skills
