- Physiotherapist required within Therapies Medicine for an acute setting (DMOPS/Medicine AUH site)
- Undertake timely physiotherapy assessments on DMOPS and Medicine wards
- Determine discharge readiness and required physiotherapy interventions
- Participate in parallel discharge planning
- Develop and monitor individualised exercise and rehabilitation programmes
- Liaise with hospital, community and multidisciplinary teams to coordinate care
- Support patient flow and timely discharge
Working Hours: 09.00 – 17.00 (shift pattern)
