Conduct comprehensive physiotherapy assessments of adult patients in community settings.

Develop, implement and evaluate personalised physiotherapy programmes focused on falls prevention and mobility.

Work collaboratively with patients, families, carers and healthcare professionals to coordinate care and support.

Act as a Best Interest Assessor where required.

Support patients with telecare interventions.

Maintain accurate and detailed clinical records in accordance with professional and organisational standards.
