  • Work as an autonomous practitioner within an integrated multi-professional team.
  • Manage patients on shared caseloads across community and care home settings.
  • Undertake assessments of patients with complex and multiple pathologies using specialist clinical reasoning skills.
  • Identify patient needs, agree appropriate goals, and provide suitable therapy interventions.
  • Provide equipment and rehabilitation programmes to support patient outcomes.
  • Take a holistic approach to therapeutic interventions, working across professional boundaries where appropriate.
  • Keep the patient at the centre of care and ensure patients and carers are involved in decision-making.

  • Remain accountable for delegated tasks and activities undertaken by Therapy Assistant Practitioners and Integrated Care Workers.
  • Prioritise patients according to clinical need and allocate or signpost tasks appropriately.
  • Maintain a consistently high standard of patient care.
  • Provide resilience and cover to other locality teams as required due to operational demands and system pressures.

About You

You will be an experienced Band 6 Occupational Therapist or Physiotherapist with the ability to work autonomously within an Intermediate Care, community, or care home environment. You will be confident assessing patients with complex and multiple conditions and using specialist clinical reasoning to determine appropriate rehabilitation and therapy interventions.

You will have a patient-centred approach and be comfortable working across professional boundaries. You will also be confident supporting colleagues, junior staff, students, and delegated staff within an integrated multidisciplinary environment.

Requirements

  • Relevant professional qualification in Occupational Therapy or Physiotherapy.
  • Current HCPC registration.
  • Experience working with older people and/or adults within a community, rehabilitation, Intermediate Care, or care home setting.
  • Experience assessing patients with complex and multiple pathologies.
  • Strong clinical reasoning and assessment skills.
  • Experience developing and delivering rehabilitation programmes.
  • Experience providing or recommending appropriate equipment to support independence and rehabilitation.
  • Ability to work autonomously within an integrated multi-professional team.
  • Ability to prioritise caseloads according to clinical need.
  • Experience supporting, supervising, or educating junior staff, students, or non-registered colleagues.
  • Strong communication and multidisciplinary working skills.
  • Commitment to delivering high-quality, patient-centred care.
