Job Overview

 
We are seeking an experienced and motivated Senior Occupational Therapist or Physiotherapist (Band 6) to join Sirona Care and Health based in Clevedon, covering the surrounding areas of Portishead, Nailsea and nearby communities. This role is community-based and focuses on supporting safe hospital discharge, preventing hospital admissions, and managing patients with frailty, falls, and neurological or orthopaedic needs.  

This post offers an exciting opportunity to work as part of a multidisciplinary team delivering high-quality, patient-centred care within a community setting. You will contribute to both urgent and planned therapy care, supporting individuals to regain and maintain independence in their own homes.  
 

Key Responsibilities

 

  • Provide comprehensive community-based physiotherapy or occupational therapy assessments and interventions to adults with diverse physical, functional, and cognitive needs.
  • Support patients within the Discharge to Assess (D2A) Pathway 1, ensuring smooth transitions from hospital to home.
  • Work proactively to reduce hospital admissions by promoting early intervention and self-management strategies.
  • Manage caseloads effectively, prioritising urgent referrals while maintaining continuity of care for planned interventions.
  • Deliver rehabilitation programmes for patients with neurological, orthopaedic, frailty, and falls-related conditions.
  • Collaborate closely with GPs, nurses, social workers, and community teams to provide holistic, integrated care.
  • Conduct home and environmental assessments to recommend adaptations, equipment, or mobility aids where necessary.
  • Encourage and educate patients and carers to promote independence and wellbeing.
  • Maintain accurate clinical records in accordance with professional and NHS standards.
  • Participate in clinical supervision, service meetings, and contribute to ongoing service improvement initiatives.

 

About You

 
The ideal candidate will be an experienced, adaptable, and proactive Occupational Therapist or Physiotherapist with a passion for community rehabilitation and integrated care. You’ll have the confidence to work independently while being an integral part of a supportive multidisciplinary team.  

You will possess excellent clinical reasoning, assessment, and planning skills, with a focus on patient-centred outcomes and evidence-based practice. Your compassionate approach and ability to manage a diverse caseload will be key to delivering effective, high-quality care in this rewarding role.  
 

Requirements

 

  • BSc (Hons) in Occupational Therapy or Physiotherapy (or equivalent qualification).
  • HCPC Registration as an Occupational Therapist or Physiotherapist.
  • Significant post-qualification experience (minimum Band 6 level preferred) within community or acute settings.
  • Experience supporting hospital discharge, D2A pathways, frailty, falls prevention, and rehabilitation care plans.
  • Knowledge of relevant health and social care frameworks and legislation.
  • Strong communication and interpersonal skills for effective MDT collaboration.
  • Excellent time management and organisational skills.
  • Ability to work independently and use initiative in dynamic situations.
  • Full UK driving licence and access to a vehicle (essential for community visits).
  • Commitment to ongoing professional development and maintaining HCPC standards.

 

Working Hours & Conditions

 

  • Hours: 37.5 hours per week (flexible 4–5 days per week)
  • Days: Monday to Friday
  • Contract: Locum (initial 6 months, ongoing)
  • Salary: £Negotiable (depending on experience)
  • Base: Clevedon – covering Portishead, Nailsea, and surrounding areas
