We are seeking an experienced and motivated Band 6 Frailty Physiotherapist to join the Betsi Cadwaladr University Health Board team at Ysbyty Gwynedd, Bangor, within the North Wales NHS. This locum role involves working across the frailty service and covering medical wards, providing high-quality physiotherapy input for patients with complex medical and rehabilitation needs. The post requires a clinician with strong assessment, treatment, and discharge planning skills who can work effectively within a multidisciplinary hospital environment.

Key Responsibilities

    • Deliver comprehensive physiotherapy assessments and evidence-based treatments to patients within frailty and medical ward settings.

    • Provide tailored interventions focused on maximising mobility, function, and independence for patients with acute and chronic medical conditions.

    • Contribute to discharge planning, ensuring patients receive appropriate rehabilitation or onward referral to community services.

    • Collaborate closely with the multidisciplinary team, including occupational therapists, nurses, doctors, and social care professionals, to provide holistic care.

    • Use clinical reasoning and outcome measures to evaluate progress and adapt treatment plans as necessary.

    • Promote safe patient handling, mobility, and exercise practice in line with NHS policies and best practice guidelines.

    • Maintain accurate and professional clinical records in accordance with HCPC and local governance standards.

You should be adaptable, confident in managing a varied caseload, and motivated to support patients in achieving their functional goals and maintaining independence.

    Requirements

    •
      • Degree or diploma in Physiotherapy and current registration with the Health and Care Professions Council (HCPC).

      • Proven NHS experience working in medical, frailty, or rehabilitation settings.

      • Strong understanding of physiotherapy principles within acute and subacute environments.

      • Excellent assessment, treatment planning, and discharge management skills.

      • Ability to work effectively within a multidisciplinary team and communicate clearly across professions.
