- Reablement Assistant required
- Support service users to meet physical, social and emotional needs in line with agreed care plans
- Promote independence and enhance daily living skills
- Must hold an up-to-date classroom Moving & Handling certificate (online only not accepted)
- Assist service users with tasks outlined in individual support plans
- Work with individuals to improve and maintain daily living skills using appropriate equipment where required
- Liaise with other professionals to minimise risk and enable individuals to remain in their own homes
- Work as part of an integrated team of allied health and social care professionals
- Participate in assessments and reviews as required
- Promote dignity, empowerment, independence and respect for cultural, spiritual and religious needs
- Maintain accurate records and report significant changes to the Line Manager
- Attend supervision sessions and contribute flexibly to changing service needs
- Support service users in activities and recreational pursuits
- Carry out duties in accordance with City Council policies and procedures
Working Hours: 35 hours per week, Monday to Friday, 09.00 – 16.00
