Job Title: Reablement Officer
Location: London Borough of Newham

Job Details:
- Reablement Officer required to start ASAP within the Adult Reablement Team
- Initial 3-month contract
- Support adults to regain independence and reduce or prevent ongoing care needs
- Deliver structured reablement and/or rehabilitation programmes in individuals’ own homes
- Work as part of an integrated intermediate care/reablement multidisciplinary team
- Contribute to clear, accurate and person-centred care and support planning documentation
- Maintain high-quality records in line with statutory and organisational requirements
- Car driver preferred
- Hybrid working – 2–3 days per week for visits
Essential Skills/Experience:
- Experience in Adult Social Care Reablement and/or Rehabilitation
- Proven experience working effectively within a Multidisciplinary Team (MDT)
- Strong communication skills and ability to collaborate across professional boundaries
- Experience contributing to person-centred care planning and documentation
- Confident IT user, ideally with experience in health and/or social care systems
Working Hours: Monday to Friday
