Reablement Assistant (Locum)
Location: Liverpool City Council – Granby Hub

About the Role
The Granby Hub is a 30-bed Intermediate Care and Reablement unit located in central Liverpool, supporting adults to regain, improve and maintain their independence. We are seeking a dedicated Reablement Assistant to join this essential service, working with individuals, families and a range of agencies to ensure coordinated, person-centred support.
Main Responsibilities
- Meet the physical, emotional and social needs of service users as outlined in their care plans, promoting independence and daily living skills.
- Assist service users with agreed tasks, including mobility, personal care and household activities.
- Support individuals to improve and maintain essential life skills, utilising appropriate equipment where required.
- Liaise with other professionals to minimise risk and enable individuals to remain safely in their own homes.
- Encourage service users to maximise independence in skills such as cooking, household maintenance, laundry and personal health management.
- Work collaboratively as part of an integrated health and social care team.
- Administer medication where required (community-based roles only).
- Support service users in implementing person-centred support plans and participate in assessments and reviews.
- Promote dignity, empowerment and respect for cultural, spiritual and religious needs.
- Hold keys to service users’ homes where appropriate and follow correct security procedures.
- Maintain accurate records and report any changes in service user circumstances to the Line Manager.
- Attend regular supervision sessions and required staff training.
- Work flexibly across a 7-day service (08:00–22:00), adapting to changing service needs.
- Support service users in social, recreational and therapeutic activities, including hydrotherapy, rebound therapy and onsite café involvement.
- Adhere to all City Council policies including health and safety, equal opportunities and anti-discriminatory practice.
- Demonstrate social value by using resources responsibly, reducing waste and supporting wider community benefits.
Skills & Experience
- Essential: NVQ Level 3 in Health and Social Care, or willingness to work towards it.
- Experience working in an adult social care setting.
- Experience supporting individuals to enhance their independence.
- Desirable: Manual Handling certification, experience with person-centred care approaches.
Working Hours
- Flexible shifts between 08:00 and 22:00, on a rota across 7 days.
