Brief Job Overview

 
An exciting opportunity has arisen for an experienced Senior Occupational Therapist or Physiotherapist (Band 6) to join Sirona Care and Health in Clevedon. This locum role is ideal for a motivated and autonomous therapist looking to make a real difference in community rehabilitation and hospital discharge pathways across North Somerset.  

You will play a key role in supporting hospital discharge, preventing readmissions, and providing rehabilitation for patients with frailty, neurological, and orthopaedic needs. The role involves a blend of urgent and planned therapy interventions within patients’ homes and community settings, supporting the Discharge to Assess (D2A) Pathway 1 where applicable.  
 

Key Responsibilities

 

  • Deliver community-based assessments and interventions to support patients’ recovery and promote independence at home.
  • Support safe hospital discharges and help prevent unnecessary hospital admissions through timely, person-centred therapy.
  • Provide rehabilitation and reablement for individuals with frailty, neurological, or orthopaedic conditions.
  • Work as part of a multidisciplinary team including nurses, social workers, and other allied health professionals to coordinate care.
  • Develop and implement individualised care and treatment plans that meet patients’ needs and goals.
  • Recommend and arrange equipment and adaptations to enhance safety and independence at home.
  • Maintain accurate clinical records in line with professional and NHS standards.
  • Participate in service meetings and contribute to continuous quality improvement initiatives.

 

About You

 
You are an enthusiastic and experienced Occupational Therapist or Physiotherapist with strong clinical reasoning and excellent interpersonal skills. You thrive in a community-based environment and are confident working autonomously while collaborating with a wider multidisciplinary team.  

You are passionate about promoting independence, preventing hospital admissions, and ensuring patients receive the right care in the right place at the right time.  
 

Requirements

 

  • BSc (Hons) or Diploma in Occupational Therapy or Physiotherapy.
  • HCPC registration (Occupational Therapist or Physiotherapist).
  • Significant post-qualification experience in community rehabilitation, frailty, or hospital discharge settings.
  • Experience working within multi-disciplinary teams and with the D2A (Discharge to Assess) model.
  • Strong understanding of falls prevention, neuro, and orthopaedic rehabilitation approaches.
  • Excellent assessment, planning, and communication skills.
  • Ability to manage a busy and varied caseload independently.
  • Valid UK driving licence and access to a car for community visits.

 

Contract & Working Pattern

 

  • Start Date: ASAP
  • Hours: 37.5 hours per week (flexible, 4–5 days per week)
  • Days: Monday to Friday
  • Contract: Locum, initial 6-month duration (with potential extension)
  • Location Coverage: Clevedon, Portishead, Nailsea, and surrounding areas (approx. 20 minutes from Bristol)
  • Salary: £Negotiable (dependent on experience and banding)
