Reablement Assistant (Locum)

 

Location: Liverpool City Council – Granby Hub

Area: Merseyside Social Services

Rate: £19.44 per hour (umbrella, negotiable)

About the Role

 

Liverpool City Council is seeking an experienced Reablement Assistant to join the Intermediate Care and Reablement Service based at the Granby Hub – a 30-bed reablement unit located in central Liverpool. The service supports individuals to regain and maintain independence through structured enablement, partnership working and personalised support plans.

 

Main Responsibilities

 

  • Meet the physical, social and emotional needs of service users in line with agreed care plans to promote independence and enhance daily living skills.
  • Assist service users with tasks identified in their support plans.
  • Support and encourage individuals to improve and maintain everyday activities, using appropriate equipment when required.
  • Liaise with multi-disciplinary professionals to minimise risk and enable service users to remain in their own homes where possible.
  • Encourage development of key life skills including kitchen tasks, household maintenance, personal care routines and self-management of health needs.
  • Work in partnership within an integrated team of health and social care colleagues.
  • Administer medication when required (for community-based work).
  • Support service users in following individualised person-centred support plans and contribute to assessments and reviews.
  • Promote dignity, independence and empowerment, respecting cultural, spiritual and religious needs.
  • Hold keys to service users’ homes when necessary, ensuring security procedures are followed.
  • Maintain accurate records and report any significant changes to the Line Manager.
  • Attend supervision sessions and participate in mandatory training and ongoing development.
  • Support and participate in recreational, educational, therapeutic and social activities, including hydrotherapy and rebound therapy.
  • Adhere to council policies, including equal opportunities and anti-discriminatory practice.
  • Work flexibly across locations within the city boundary and adapt to the evolving needs of the service.

 

Additional Responsibilities

 

  • No supervisory or line management duties.
  • Responsible for managing resources effectively, reducing waste and ensuring compliance with statutory requirements.
  • Provide safe support with personal care, including use of manual handling equipment.

 

Skills & Experience Required

 

  • NVQ Level 3 in Health and Social Care, or willingness to work towards this qualification.
  • Manual handling experience (certificate desirable).
  • Experience working in an adult social care or reablement setting.
  • Ability to promote independence through structured programmes.
  • Experience delivering person-centred care and following reablement plans.
  • Good written and verbal communication skills.
  • Ability to work flexibly across a rota covering 8am–10pm, 7 days per week.

 

Working Hours

 

  • Flexible rota between 08:00 and 22:00, 7 days a week.
