Conduct comprehensive assessments of functional abilities and housing-related rehabilitation needs.

Develop, implement and evaluate tailored intervention plans in line with best practice and individual goals.

Work in partnership with service users, carers and professionals to coordinate care and facilitate access to community resources.

Maintain accurate and timely clinical records in accordance with organisational and professional standards.

Provide advice, education and training to support safe living and occupational engagement.

Contribute to service development and quality improvement initiatives within rehabilitation and housing occupational therapy services.
