Job details: To start asap. Community Mental Health Practitioners hold a caseload in the team and also play an active role in the weekly Duty system. The role involves completing Health and Social Care Needs assessments/ Access and Assessment tool, Risk Assessments, Dialog+ interventions, completing Safety Plans, Care Plans, monitoring patient’s mental state, arranging and attending medical reviews for patients with our team of Doctors. Completing referrals to relevant care pathways, offering support where required to identified carers and signposting for support where required. Working hours: Full time. Monday to Friday, 09.00 – 17.00.
