HCRG Care Group is seeking an experienced and enthusiastic Band 6 Occupational Therapist to join its community-based team. The role offers a supportive environment for professional development and the chance to make a real impact on the lives of individuals across local communities.

Key Responsibilities:

  • Undertake comprehensive occupational therapy assessments and deliver evidence-based interventions to support patients’ independence and wellbeing within their own homes or community settings.

  • Manage a complex caseload autonomously, using clinical reasoning and judgement to develop tailored treatment and rehabilitation plans.

  • Work collaboratively with a multidisciplinary team to ensure coordinated care that meets patients’ physical, psychological, and social needs.

  • Support patients in setting and achieving meaningful goals to improve function, prevent hospital admission, and facilitate timely discharge.

  • Contribute to the supervision and development of junior staff, students, or support workers as required.

  • Maintain accurate clinical records and participate in audits, service evaluations, and quality improvement initiatives in line with professional standards and organisational policies.
