This role involves working with children and young people with complex needs and their families, providing statutory support, safeguarding, and long-term care planning to improve outcomes and promote independence.
Key Responsibilities:

Undertake statutory assessments under the Children Act 1989 and Children and Families Act 2014 for children with disabilities.

Develop and review care and support plans tailored to individual needs, ensuring inclusion and wellbeing.

Provide safeguarding interventions, risk assessments, and contribute to child protection and CIN plans.

Liaise with education, health, short breaks services, and voluntary agencies to coordinate care.

Prepare reports for statutory reviews, court, and multi-agency meetings, maintaining accurate and timely case records.
