View JobWe are seeking a skilled and compassionate Children with Disabilities (CWD) Social Worker to support children and young people with complex needs and their families. This role focuses on improving quality of life, promoting independence, and ensuring access to essential services and safeguarding support. The ideal candidate will be experienced in working with children with disabilities and committed to inclusive, person-centred practice.
Key Responsibilities:

Assess the needs of children with disabilities and develop tailored care and support plans in partnership with families and professionals.

Coordinate multi-agency support to ensure access to education, health services, respite care, and community resources.

Support transitions from childhood to adulthood, including education and independent living planning, ensuring continuity of care.

Safeguard vulnerable children by identifying risks, managing protection plans, and acting in accordance with statutory guidelines.
