Key Responsibilities:

Undertake statutory assessments and care planning for children and young people with disabilities.

Work closely with families, schools, health services, and care providers to coordinate tailored support packages.

Carry out Child in Need, Child Protection, and Looked After Children duties as required.

Promote a strengths-based, person-centred approach that empowers families and supports inclusion.

Maintain detailed and up-to-date records, and contribute to reviews, panels, and multidisciplinary meetings.
