Act on behalf of Bournemouth, Christchurch and Poole Council to carry out Care Act 2014 assessments, supporting adults to maintain independence, wellbeing, and dignity in their daily lives.

Manage a varied caseload including older adults, individuals with physical or learning disabilities, and those with mental health needs, delivering personalised and timely interventions.

Develop and review person-centred care and support plans, ensuring they are strengths-based, outcome-focused, and co-produced with service users and their carers.

Lead and contribute to safeguarding enquiries under Section 42 of the Care Act, working closely with other agencies to protect adults at risk of abuse, neglect, or exploitation.

Maintain high-quality, accurate records and reports, complying with legal and professional standards, and contributing to best-interest decisions, Continuing Health Care assessments, and Mental Capacity Act processes.

Engage in professional supervision and development, contributing to team learning, reflective practice, and continuous service improvement within Adult Social Care at BCP Council.
