  • Undertake strengths-based assessments in line with the Care Act 2014, on behalf of Darlington Borough Council, to identify eligible needs and promote independence, wellbeing, and personal choice.

  • Manage a caseload of adults with varying levels of need, including older people, individuals with physical or learning disabilities, and those experiencing mental health issues.

  • Develop person-centred care and support plans, working closely with service users, carers, families, and partner agencies to ensure effective, coordinated support.

  • Safeguard vulnerable adults by recognising and responding to abuse, neglect, or exploitation, and participating in Section 42 Enquiries as appropriate.

  • Maintain accurate and timely records in line with council policies and legal frameworks, including the preparation of reports for case reviews, best interest meetings, and court proceedings.

  • Contribute to service development by engaging in supervision, reflective practice, training, and team meetings, promoting continuous learning and quality improvement within Adult Social Care.
