Deliver statutory adult social care duties on behalf of Cheshire East Council, supporting individuals under the Care Act 2014 to achieve improved independence and wellbeing.

Carry out strengths-based assessments, reviews, and support planning for adults with a range of needs, including older people, physical disabilities, and learning disabilities.

Manage a diverse caseload, balancing crisis intervention, long-term care planning, and preventative approaches in line with best practice.

Undertake Section 42 safeguarding enquiries, ensuring timely identification and mitigation of risks in collaboration with multi-agency partners.

Apply the Mental Capacity Act 2005 in day-to-day practice, including completing capacity assessments and participating in best interest decisions.

Maintain accurate records, participate in supervision and training, and contribute to continuous service improvement across adult social care.
