Deliver statutory adult social care services for Derbyshire County Council, supporting adults with care and support needs under the Care Act 2014.

Undertake strengths-based assessments, reviews, and personalised support planning for older adults and individuals with physical, learning, or mental health needs.

Manage a diverse caseload, including safeguarding concerns, hospital discharges, and complex long-term care arrangements.

Conduct Section 42 safeguarding enquiries, working with partners to assess and manage risks, ensuring the safety and wellbeing of vulnerable adults.

Apply the Mental Capacity Act 2005, completing capacity assessments and best interest decisions where required.

Maintain accurate case records, engage in supervision and training, and contribute to service development through reflective practice and quality assurance activities.
