Community Occupational Therapist – Band 6 (Locum)

Job Overview

We are recruiting an experienced Band 6 Community Occupational Therapist to join Leeds Community Healthcare NHS Trust within the West Yorkshire NHS. This locum role focuses on supporting adults in community settings, providing occupational therapy assessments and interventions that promote independence, safety, and wellbeing. You’ll work as part of a multidisciplinary team, delivering person-centred care across a varied caseload.

 

Key Responsibilities

  •   
  • Carry out occupational therapy assessments in community settings, identifying functional needs and developing personalised intervention plans.
     

  • Deliver evidence-based rehabilitation to support individuals in maintaining independence within their homes and local environments.
     

  • Work collaboratively with multidisciplinary colleagues to support integrated, holistic care across community pathways.
     

  • Monitor progress and adjust treatment plans as required, ensuring outcomes remain aligned with patient goals.
     

  • Provide clear documentation and maintain accurate clinical records in line with HCPC and NHS standards.
     

  • Support safe and effective discharge planning and onward referrals where needed.

 

About You

 

You are a skilled and adaptable Occupational Therapist with experience working in community or rehabilitation settings. You bring strong assessment abilities, sound clinical reasoning, and a flexible approach to managing a diverse caseload. Your commitment to supporting adults in their own homes and working collaboratively with colleagues will make you a valuable part of the community therapy team.

 

Requirements 

  • Degree or diploma in Occupational Therapy and registration with the Health and Care Professions Council (HCPC).
     

  • Previous NHS experience, ideally within community-based services.
     

  • Strong assessment, planning, and intervention skills.
     

  • Excellent communication, documentation, and organisational abilities.
     

  • Ability to work independently and manage a varied community caseload.
