- Maintain accurate and timely electronic case records using Liquidlogic and, where applicable, the ELMS equipment portal.
- Work collaboratively with health professionals, housing services, carers, and partner agencies to achieve positive outcomes.
About You
You will be a HCPC-registered Occupational Therapist with experience working within Adult Social Care and managing a diverse community caseload. You will have excellent assessment and clinical reasoning skills, together with experience recommending equipment, adaptations, and housing solutions that maximise independence.
You will be confident working independently, managing competing priorities, and building effective working relationships across multidisciplinary teams.
