Social Care Assessor – Integrated Learning Disabilities Service (Transitions)

Job Overview

We are looking for an enthusiastic and compassionate Social Care Assessor to join the Integrated Learning Disabilities Service (ILDS) within North London Social Services. This rewarding locum opportunity focuses on supporting young people with learning disabilities as they transition from Children’s Services into Adult Social Care.

Working alongside the Senior Practitioner leading the Transition Project, you will play a vital role in ensuring young people receive the support they need to achieve positive outcomes in adulthood. You will undertake strengths-based assessments, develop person-centred support plans, review care packages, and work collaboratively with families and professionals to deliver seamless transition planning.

Key Responsibilities

  • Undertake strengths-based social care assessments for young people transitioning into Adult Social Care.
  • Develop and review person-centred support plans and care packages.
  • Support young people and their families through the transition process, promoting independence and positive wellbeing.
  • Work collaboratively with schools, health professionals, social workers and partner agencies to deliver coordinated care.
  • Participate in safeguarding enquiries and contribute to the team’s duty service.
  • Promote a person-centred approach that empowers individuals to achieve their goals and maximise their potential.
  • Maintain accurate case records and ensure assessments and reviews are completed within agreed timescales.
  • Work as part of a fully integrated multidisciplinary learning disabilities service.

About You

You will be passionate about supporting people with learning disabilities and committed to delivering high-quality, strengths-based social care. You will have excellent communication and partnership-working skills, enjoy working within a multidisciplinary team, and be dedicated to helping young people achieve successful transitions into adult life.

Requirements

  • Experience undertaking social care assessments within Adult Social Care or Learning Disability Services.
  • Knowledge of supporting young people with learning disabilities during transition to Adult Services.
  • Experience developing person-centred support plans and reviewing care packages.
  • Understanding of safeguarding legislation and best practice.
  • Excellent communication and relationship-building skills with individuals, families and professionals.
  • Ability to work collaboratively within a multidisciplinary team.
  • Strong organisational skills with the ability to manage and prioritise a varied workload.
  • Commitment to strengths-based practice and promoting independence, wellbeing and inclusion.

Contract & Salary

  • Role: Social Care Assessor – Integrated Learning Disabilities Service (Transitions)
  • Area: North London Social Services
  • Job Type: Locum
  • Contract: Initial 3-month assignment with ongoing potential and the possibility of a longer-term fixed-term opportunity, subject to service needs.
  • Salary: £23.87 per hour.
  • Working Pattern: Hybrid working – 3 days office-based and 2 days working from home.
  • Wednesday is a mandatory office day.
