Job details:  Community. To start asap for 3 months initially. The Extended Primary Care Services enable the provision of multi-disciplinary care and support to vulnerable adults, in a range of community settings. Patients are referred primarily due to a physical disability and have complex medical and social needs. Nursing, therapy and support staff liaise closely with local GPs to provide the majority of community health services for adults living at home. Working hours: Full time.
