Job details:  Integrated Health Team. To assess the Physiotherapy needs of a defined patient group and establish and evaluate appropriate treatment plans. This includes chronic, acute and palliative care within own competencies, recognising own limitations and seeking advice when necessary. The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will supervise pre-registration students/staff. They will have a commitment to collaborative working and actively supporting and liaising with other health and social professionals and agencies. Skills/experience: Physiotherapy degree or equivalent, Health Professions Council Registered. Good knowledge of current best practice in Physiotherapy in acute, physical health, community, and rehab setting. And be able to prescribe equipment to facilitate safe discharge process. Service operational day is from 07:00-22:00 and comprises four visit patterns: - 09:00-17:00, Flexible as per service requirements. Working hours: shifts, 09.00 – 17.00.
