Job details:  Community Physiotherapist required for general physical rehabilitation. Adult community rehabilitation team. 12 weeks. To undertake specialised assessment and provide interventions to adults with complex medical presentations as an autonomous lone practitioner in the community setting. To monitor, evaluate and modify treatment and management for patients in order to measure progress and ensure effectiveness of intervention. Ensure that every patient involved in the rehabilitation process has meaningful and relevant timely goals that are challenging but achievable. To ensure accurate and evaluative records of treatment and patient care are maintained in accordance to professional and service standards. To be responsible for own practice and accountable for all aspects of physiotherapy treatment and care delivered to patients accepting responsibility for a designated caseload ensuring prioritisation where appropriate. To have specialist knowledge and to ensure the use of outcome measures to measure the efficacy and effectiveness of treatment. Working hours: 37.5 hours per week
