Job details:  To start asap. Hybrid - Working from home/NPTH. Community Physiotherapist required to support the virtual ward fracture discharge service. to provide initial physiotherapy assessment in patient home following discharge from Hosp due to ortho injury. To provide ongoing physiotherapy intervention as needed to support patient's rehabilitation post injury. Required to work within an MDT to provide holistic support to patients on the fracture discharge pathway. Working hours: Full time. 08.30 – 16.30.
