Job details:   Rehab Community. An urgent falls response service is beng set up and requires an experienced OT to support this new pilot. Ideal candidate would have extensive experience of community rehabilitation and preferably experience of working in a Rapid Response/ Falls response service. The post holder will be required to work as part of and jointly lead a team of therapist and support worker. The service will respond to urgent referrals for patients who have fallen. The ability to carry out holistic assessment and implement robust plans to support patients to remain at home is essential. Car driver is essential as staff would be expected to transport some equipment as part of the role. Working hours: Full time. 37.5 hours, over 7 days a week on a rota.

Job details:  Intensive Rehab Service. OT required to work in an After COVID 19 community rehab team. Post holder will be working as part of the MDT which involves both Community and Acute partners. They will be managing a complex case load of COVID patients in the community. They will assess, identify problems, set goals and plan rehab. They will liaise with the MDT of patients on going needs, progress made and discharge from case load. They will keep accurate documentation and also will contribute to the national and local data. They will take responsibility to be up to date with relevant training and COVID guidelines
