• To assess individual patient’s needs, set and implement treatment plans and long-term condition management for patients linking with primary, secondary and tertiary care, using a coordinated approach.

• To contribute to the development of the Occupational Therapy role within the Occupational Therapy Service by working across boundaries and interagency working.

• As part of a team, initiate research-based practice and monitor the quality of the service being provided.
