The post holder is involved with the full assessment of patients referred into the Long COVID Tier 3 service determining the suitability to provide a specialist home care package for patients with acute exacerbations of Chronic Obstructive Pulmonary disease and Bronchiectasis in the community as an alternative to hospital admission. To manage a caseload of clients with these chronic diseases using evidence based and client-centred principles to assess, plan, implement and evaluate interventions.
The post holder is expected to carry out all relevant forms of care with direct supervision within a defined clinical area. The post holder will provide a highly specialised physiotherapeutic input of acute patients with Respiratory conditions.
To provide advice, supervision, and training to students, Actrite colleagues and other medical and educational personnel as directed by service developments.
To work in the community as a lone practitioner in line with the Trust’s Lone Worker Policy, providing chest physiotherapy in the client’s own home. Care delivery according to changing health care needs and varying levels of complexity. To ensure that patients and their carers are involved in the planning, goal setting and reviewing of their treatment plans whenever possible.
To provide self-management plans as appropriate for all patients. To provide training and support for patients and their carers where appropriate. Empower others and demonstrate an ability to make effective clinical decisions based on recent evidence. Assessment and management of Bronchiectasis patient.
