JD available on request: The Community Rehabilitation and Short-Term Assessment Team (CRSTAT) provides rehabilitation, including Occupational Therapy and Physiotherapy, to adult client’s living in the borough of Greenwich, across various community settings and in their homes. This service is integrated with social care. The physiotherapist will work as part of the multidisciplinary Integrated Team to meet the rehabilitation needs of the service users and their carers, enhancing independence, safety and quality of life, maintaining people in their own home environments in conjunction with Social Care colleagues.

• To provide Physiotherapy input to clients referred to CRSTAT.
• To practice within the multidisciplinary team as an autonomous practitioner, ensuring the highest possible standards of practice
• To work in line with the Trust objectives for community practice.

The Specialist Speech and Language Therapist will have a delegated specialist clinical responsibility for an agreed clinical area across the service and dissemination of information and resources as appropriate.
• You will assess the need for, develop and implement evidence-based speech and language therapy interventions to children with a wide range of speech, language and communication difficulties.
• You will support in the training and development of colleagues within the team.

• The service is a multidisciplinary rehabilitation team (MDT), specialising in working with people who have Neurological Conditions. It is community-based, providing a specialist rehabilitation service to people living in the Royal London borough of Greenwich, in a location that suits their rehabilitation need, which includes their place of residence, day centres, or place of work. 
• This post will enable the successful post-holder to develop their skills across 3 caseloads:

• Neuro Rehabilitation Team
• Early supported discharge for Stroke
• Community SLT (non-neuro)
• We offer extensive support both from the SLT team and the wider multidisciplinary team. There is a well-developed supervision structure in place. Our service actively encourages ongoing professional development and you will be given the opportunity to attend a wide variety of internal and external training. You will have the opportunity to be involved in appraisal, development and supervision of junior staff and students. 
• You will have the opportunity to refer, attend and develop competency in videofluoroscopy and FEES. You will also have the opportunity to facilitate communication groups and carry out innovative rehabilitation programmes including ICAP, EMST and sEMG.

• The Physiotherapist will work as a part of the multi-disciplinary Rapid Response team including health and social services colleagues to meet the needs of patients referred to the team, ensuring the highest possible standards of practice. The post holder will be expected to deputise in the absence of the Deputy Managers for the clinical management of the care of patients referred to the team.
• The Rapid Response Team aims to prevent inappropriate admissions to acute hospital wards, unnecessary Emergency Department attendances from the community and facilitate early discharges from hospital by acting as the gateway to Community Services within the Borough of Bexley. The post holder will conduct specialist Physiotherapy assessments for patients in Emergency Departments, Acute Medical Units and in the community. The team provides an integrated multi-professional assessment of health and social care needs. The post holder will assess and lead on recommending admission to intermediate care facilities for rehabilitation or refer to integrated health and social care community services for appropriate intervention, community follow up and case management. The post holder will also provide equipment necessary to maintain clients at home when urgently required following assessment.

Working hours: Monday to Sunday, 8-4pm or 12-8pm. This role requires 3 shifts together per week and 1 weekend in 4 to be worked.

• The post will involve providing specialist physiotherapy intervention to children 0-19.? The caseload will be mixed, to include MSK, complex neurological conditions, DCD, baby and gait clinics and mainstream and special schools.? ? 

• You will be working as part of a team delivering, assessments, interventions, group sessions and liaison within education settings.? 

• Services will be provided across health, education and community care settings including seeing children in their own home, school and in hospital, on the Child Development Centre, Special schools, aiming to maximise the potential of children with a wide range of disabilities and conditions through the development of comprehensive care plans.? ? 

• There may be an option to have a more focused caseload if candidates have a specific clinical speciality.? 

Working hours: 5 days per week. 08.30-16.30 or 09.00 – 17.00.
