The role is primarily office and community based, with approximately one day per week working from home.
Key Responsibilities

- Complete strengths-based assessments, reviews, and care and support planning in accordance with the Care Act 2014.
- Carry out specialist functional assessments, including activities of daily living, motivation, communication, sensory processing, employment readiness, community access, and meaningful occupation.
- Develop person-centred interventions that promote recovery, wellbeing, independence, and positive risk-taking.
- Complete risk assessments and formulate risk management plans that balance safety with independence.
- Undertake safeguarding enquiries and contribute to safeguarding plans in line with multi-agency safeguarding procedures.

- Liaise with Adult Social Care Occupational Therapy colleagues regarding manual handling assessments, specialist equipment, adaptations, and Disabled Facilities Grants (DFGs).
- Promote the use of assistive technology and Telecare to maximise independence.
- Maintain accurate electronic case records and produce high-quality assessment reports in accordance with statutory and professional standards.
- Participate in multidisciplinary meetings, service development, and continuous professional development activities.
- Provide mentoring, coaching, and informal support to less experienced colleagues where appropriate.
About You
You will be a HCPC-registered Occupational Therapist with experience working within Adult Social Care and Mental Health services. You will have excellent knowledge of strengths-based practice, the Care Act 2014, safeguarding, risk management, and recovery-focused interventions.
You will be confident undertaking complex functional assessments, working autonomously within the community, and building effective relationships with service users, carers, and multidisciplinary professionals. Experience of housing adaptations, assistive technology, and integrated working would be highly advantageous.
Requirements
